It is the goal of the Psychosocial Department at Belmont Crossing to provide person-centered treatment that promotes independence and the motivation to thrive in the community.
All residents at Belmont Crossing have the opportunity to receive individual counseling to create and achieve personal recovery goals alongside our Qualified Mental Health Professionals.
Belmont Crossing provides residents the opportunity to share and work on recovery goals in a group setting surrounded by their peers.
We are dedicated to working with each resident to start planning for their future in the Chicago community and beyond. We are closely connected to Third Party case managers who are a part of the Williams Consent Decree (aka the Moving On Program).
Person-Centered Case Management
Belmont Crossing's Psychosocial Rehabilitation Department provides ongoing, person-centered case management and treatment planning. All treatment options are based on the individual resident and their needs to prosper.
